Treatment overview

A haploidentical bone marrow transplant is an allogeneic stem cell transplant in which the donor shares only half of the patient's HLA immune markers — typically a biological parent, child, or sibling. Because nearly every patient has at least one such relative, this approach has made transplant accessible to many who cannot find a fully matched donor. The procedure involves conditioning chemotherapy (and sometimes radiation) to eliminate residual disease and suppress the immune system, followed by infusion of the donor's stem cells on 'day zero.' What distinguishes haploidentical transplant from other allogeneic transplants is the use of post-transplant cyclophosphamide on days +3 and +4, which selectively eliminates immune cells likely to cause graft-versus-host disease while sparing the engrafting stem cells. Patients remain in hospital for approximately four to six weeks while the new marrow establishes itself, followed by months of close outpatient monitoring.
