Selective dorsal rhizotomy (SDR) is a neurosurgical procedure performed on the lower spinal cord to permanently reduce spasticity in the legs of children with spastic cerebral palsy. The surgeon makes an incision in the lower back, exposes the sensory nerve roots, and — guided by intra-operative electrical testing — selectively cuts the rootlets sending abnormal signals that drive the spastic feedback loop. Because only the abnormal rootlets are divided, useful sensation and motor function are preserved. SDR is considered for children who have sufficient underlying muscle strength to benefit from reduced tone, most commonly those with spastic diplegia at GMFCS Levels II–III, typically between the ages of 3 and 10. The surgery itself is followed by many months of intensive physiotherapy, which is essential to translate the spasticity reduction into functional walking and movement gains.
