Cost of Pediatric Leukemia Treatment in India

Treatment overview

Pediatric leukemia is a cancer of the blood-forming tissue in the bone marrow, where abnormal immature white blood cells — called blasts — multiply uncontrollably and crowd out healthy red cells, white cells, and platelets. It is the most common childhood cancer, and modern medicine has made remarkable progress in treating it: childhood acute lymphoblastic leukemia (ALL), the most frequent type, now has cure rates above 85–90% in well-resourced centers. Treatment is tailored to the specific leukemia type (ALL, AML, CML, or rarer subtypes), the child's risk category, and how the disease responds to early therapy. The main treatment approaches include multi-drug chemotherapy protocols delivered in phases, targeted or immune-based therapies for specific molecular subtypes, and hematopoietic stem cell transplant (bone marrow transplant) for high-risk or relapsed disease. Treatment typically spans two to three years for ALL, or six to nine months of intensive chemotherapy for AML.

5 approaches available for this treatment / procedure:

Chemotherapy Protocol (ALL — Induction + Consolidation + Maintenance)

Multi-phase chemotherapy delivered over two to three years for acute lymphoblastic leukemia — the most frequent childhood leukemia — beginning with intensive induction and progressing to gentler home-based maintenance.

Comprehensive Investigations

Estimated cost: ~$1,000 – $2,500

Typical tests: Complete blood count (CBC) with differential and reticulocyte count, Bone marrow aspiration and biopsy with immunophenotyping (flow cytometry), Cytogenetics — karyotype, FISH panel for key translocations, Molecular testing — PCR for BCR-ABL, TEL-AML1, MLL rearrangements, MRD baseline, Lumbar puncture with CSF cytology, 2D Echocardiogram (baseline cardiac function before anthracyclines), Renal and liver function tests, uric acid, LDH, electrolytes, Chest X-ray and ultrasound abdomen/pelvis, Pediatric anesthesia clearance assessment. Done in India before admission.

Treatment / Procedure estimate

Estimated cost in India: $6,000 – $15,000

Treatment Duration

Treatment duration in India: ~6 weeks (~35 hospital visit days across the full span)

Treatment Plan Induction phase (approximately 4–6 weeks) requires near-daily hospital attendance or inpatient admission; consolidation phase (months 2–8) involves regular outpatient visits with periodic short admissions.

Usually Included in Treatment Package

  • Pediatric oncology specialist consultations throughout the treatment phase
  • Pre-treatment diagnostic workup at hospital (bone marrow studies, molecular/genetic tests, imaging, cardiac echo)
  • Multi-drug chemotherapy protocol for ALL (vincristine, asparaginase, steroids, methotrexate, 6-mercaptopurine, and…
  • Central venous line (PICC or port) insertion and care
  • Inpatient nursing, monitoring, and supportive care (transfusions, anti-infectives, anti-nausea medications)
  • Intrathecal chemotherapy and lumbar puncture procedures
  • Routine follow-up consultations and blood count monitoring during the treatment phase
  • Dietitian and supportive multidisciplinary team access during admission

Intensive Chemotherapy Protocol (AML — Multiple Cycles)

Shorter but more intensive chemotherapy delivered over approximately six to nine months in repeated inpatient cycles for acute myeloid leukemia, which requires more aggressive treatment than ALL.

Comprehensive Investigations

Estimated cost: ~$1,500 – $2,500

Typical tests: Complete blood count (CBC) with differential and reticulocyte count, Bone marrow aspiration and biopsy with immunophenotyping (flow cytometry), Cytogenetics — karyotype and FISH for AML-relevant translocations (t(8;21), inv(16), t(15;17)), Molecular testing — FLT3-ITD, NPM1, CEBPA, KIT, IDH1/2 mutations; PML-RARA for APL, Lumbar puncture with CSF cytology, 2D Echocardiogram and ECG (anthracycline baseline), Renal and liver function tests, uric acid, LDH, coagulation profile (PT/INR/APTT), HLA typing (for potential transplant planning), Pediatric anesthesia clearance assessment. Done in India before admission.

Treatment / Procedure estimate

Estimated cost in India: $10,000 – $20,000

That works out to roughly $2,000 – $4,500 per cycle across 4 cycles, plus a one-time pre-treatment work-up of about ~$1,500 – $2,500 (done once). Per-cycle figures are approximate — the headline range above covers the full course.

Treatment Duration

Treatment duration in India: ~6 months (4 cycles × 28 days, ~112 hospital visit days)

Treatment Plan AML is treated with 4–5 intensive chemotherapy cycles, each requiring extended inpatient admission (typically 3–5 weeks per cycle due to prolonged neutropenia and infection risk). There is no maintenance phase.

Usually Included in Treatment Package

  • Pediatric oncology specialist consultations throughout the treatment phase
  • Pre-treatment diagnostic workup at hospital (bone marrow studies, molecular/genetic tests, imaging, cardiac echo)
  • Intensive multi-drug AML chemotherapy protocol (cytarabine, anthracyclines, and subtype-directed agents)
  • Central venous line (PICC or port) insertion and care
  • Inpatient nursing, monitoring, and supportive care (transfusions, anti-infectives, anti-nausea medications)
  • Intrathecal chemotherapy and lumbar puncture procedures
  • Routine follow-up consultations and blood count monitoring during the treatment phase
  • Dietitian and supportive multidisciplinary team access during admission

Targeted Therapy (Philadelphia-Positive ALL / CML)

Daily oral tyrosine kinase inhibitor (TKI) medication combined with standard chemotherapy, used for children whose leukemia carries the Philadelphia chromosome — a genetic change that makes standard chemotherapy alone less effective.

Comprehensive Investigations

Estimated cost: ~$1,500 – $2,500

Typical tests: Complete blood count (CBC) with differential, Bone marrow aspiration and biopsy with immunophenotyping, Cytogenetics — Philadelphia chromosome (t(9;22)) confirmation by karyotype and FISH, Quantitative BCR-ABL PCR (p190 for ALL, p210 for CML) — baseline, ABL kinase domain mutation screening (if prior TKI exposure), Lumbar puncture with CSF cytology (for Ph+ ALL), 2D Echocardiogram and ECG, Renal and liver function tests (TKI hepatotoxicity monitoring baseline), Pediatric anesthesia clearance assessment. Done in India before admission.

Treatment / Procedure estimate

Estimated cost in India: $8,000 – $18,000

Treatment Duration

Treatment duration in India: ~6 weeks (~30 hospital visit days across the full span)

Treatment Plan TKI therapy (imatinib, dasatinib, or equivalent) is added to the backbone chemotherapy protocol and taken as a daily oral medication throughout treatment. For Ph+ ALL, induction is delivered as inpatient/frequent outpatient over 4–6 weeks, with TKI continuing through all subsequent phases.

Usually Included in Treatment Package

  • Pediatric oncology specialist consultations throughout the treatment phase
  • Pre-treatment diagnostic workup at hospital (bone marrow studies, molecular/genetic tests, imaging, cardiac echo)
  • TKI drug supply for the initial treatment phase (imatinib or dasatinib as protocol-directed) plus backbone chemotherapy
  • Central venous line (PICC or port) insertion and care
  • Inpatient nursing, monitoring, and supportive care (transfusions, anti-infectives, anti-nausea medications)
  • Intrathecal chemotherapy and lumbar puncture procedures
  • Routine follow-up consultations and BCR-ABL PCR monitoring during the treatment phase
  • Dietitian and supportive multidisciplinary team access during admission

Pediatric Bone Marrow Transplant (Allogeneic)

Replacement of the child's diseased bone marrow with healthy donor stem cells — recommended for high-risk leukemia subtypes, disease that has not responded adequately to chemotherapy, or leukemia that has relapsed.

Treatment / Procedure estimate

Estimated cost in India: $20,000 – $45,000

For more details, visit: Pediatric Bone Marrow Transplant →

CAR-T Cell Therapy (Relapsed / Refractory B-cell ALL)

A cutting-edge immunotherapy where the child's own T-cells are genetically engineered in a laboratory to recognise and destroy leukemia cells, used when B-cell ALL has come back or has not responded to prior treatments.

Treatment / Procedure estimate

Estimated cost in India: $35,000 – $60,000

For more details, visit: CAR-T Cell Therapy →

Usually Not Included

I can honestly say that my experience seeking treatment abroad was nothing short of exceptional, and it really highlighted just how disappointing my experience with the NHS had been in comparison. After facing long delays, lack of clear communication, and a general feeling of being overlooked in the UK system, making the decision to travel overseas for care felt like a last resort—but it turned out to be the best decision I could have made. From the moment we arrived, everything was handled with a level of professionalism and efficiency that immediately put me at ease. The hospital itself was spotless, modern, and incredibly well-organised. There was no sense of chaos or long waiting around; every appointment, test, and consultation was arranged promptly and ran exactly on time. It felt like the entire system was built around patient care and comfort, rather than patients having to fit into an overstretched system. The medical team were outstanding. Every doctor and nurse I interacted with took the time to listen carefully, explain things clearly, and make sure I fully understood my treatment plan. I never once felt rushed or dismissed. Instead, I felt genuinely cared for and respected, which made a huge difference during what was already a stressful time. One of the standout parts of the entire experience was the support we received from Natasha, our international client coordinator. From the very beginning, she went above and beyond to ensure everything ran smoothly. She organised appointments, helped with paperwork, arranged transport, and was always available whenever we had questions or concerns. Her communication was excellent, and she made what could have been a very overwhelming experience feel simple and manageable. Natasha didn’t just do her job—she genuinely cared. She checked in on us regularly, made sure we were comfortable, and provided reassurance every step of the way. Having someone like her there made a huge difference, especially being in a foreign country where things can easily feel unfamiliar or stressful. Overall, the level of care, attention, and efficiency we experienced abroad was on a completely different level. It’s frustrating to say, but the contrast with the NHS was stark. Where we had previously faced delays and uncertainty, we instead found clarity, speed, and genuine compassion. I would highly recommend this hospital to anyone considering treatment abroad. The quality of care, combined with the incredible support from staff like Natasha, made what could have been a difficult experience into a very positive one. I’m truly grateful for the care I received and wouldn’t hesitate to make the same decision again if needed.
